The Congress has suffered the biggest setback from Madhesh Province. Congress President Gagan Kumar Thapa himself had contested from Sarlahi-4 to increase the party's influence in the most populous Madhesh Province. However, he not only lost to Rashtriya Swayamsevak Sangh candidate Amresh Kumar Singh by a huge margin, but also won only 1 seat out of 32 seats in Madhesh Province. Congress candidate Firdosh Alam won only from Rautahat-2. He is the son of former minister Mohammad Aftab Alam.

The Congress has also received very less in proportional representation from Madhesh Province. While the National Independent Party (RSWP) received 1.18 million votes, the Congress received only 2.51 million votes.

In Sarlahi-4, where the president himself contested the election, the Rashtriya Swayamsevak Sangh (RSS) got 45,516 votes in the proportional representation. The direct candidate of the Rashtriya Swayamsevak Sangh (RSS) Amresh Kumar Singh got 35,688 votes. The Congress got even fewer votes in the proportional representation than the President Thapa. Thapa got 22,831 votes, and the party got a total of 18,081 votes in the proportional representation.

Madhesh was once considered the main base of the Congress. After the 2062/063 people's movement and the Madhes agitation, the Congress's influence in the Madhes, which was shrinking after the 2062/063 people's movement and the Madhes agitation, has been hit even harder this time. Earlier, in the 2079 election, the Congress had won 7 seats in an alliance between the Congress, the Maoists, the Unified Socialists and the Madhes-centric Democratic Socialist Party. At that time, the Congress also received more proportional votes from this constituency.

In 2079, due to the alliance, the Congress had fielded candidates in only 15 constituencies out of 32 seats. Out of those, it had won 7 seats. However, this time, the Congress has had to settle for only one seat.
