She has a Master's degree in Medicine in General Surgery. She completed her SSLC from Shahid Dharmabhakta Secondary School and 12th from Modern Indian School in Chobhar. Karki, who is well-educated, completed her MBBS from Kisht Medical College in Lalitpur and her Master's degree in Medical Education from Kathmandu University.
She is a doctor who served the citizens during the 2072 earthquake and the COVID pandemic. She volunteered in Madhesh Pradesh, Sindhupalchowk, Nuwakot and other districts. She also worked at Patan Hospital for some time. She has been raising her voice against malpractices in medical colleges since 2071/072. She was actively involved in the campaign of medical education reform activist Dr. Govinda KC. She joined the Rashtriya Swayamsevak Sangh (RSS) and was elected from Lalitpur-3 in 2079. She became the Minister of State for Health and Population in the Pushpa Kamal Dahal-led government from 2079 to 22 Magh.
Dr. Karki was elected from Lalitpur-3 in 2079, defeating Maoist candidate Pampha Bhusal, who was also the then Energy Minister, and UML candidate Amrit Khadka. Toshima received 31,136 votes. Her nearest competitor Amrit Khadka of UML received 12,963 votes. Bhusal of the Maoist received only 8,769 votes. Dr. Karki reached the voters with the slogan 'Healthy Nepali, Happy Nepali'. Karki's candidacy was initially rejected by the Election Commission. The Election Commission had rejected Karki's candidacy, saying that he had taken a position of profit as a member of the Nepal Medical Council and that such a person would be ineligible to be a candidate. Hearing Karki's petition, which had reached the Supreme Court against it, the Election Commission gave an interim order to uphold his candidacy.
After being removed from the post of Minister of State for Health, she used to raise her voice on the issues of quality healthcare and medical education for the citizens by being in the Education, Health and Information Technology Committee of Parliament. Dr. Karki had taken to the streets in support of the Gen-G movement on 23 and 24 Bhadra.
Dr. Pawan Sharma, former rector of Patan Institute of Health Sciences, claimed that Karki would be excellent for the future in both the medical field and politics. ‘When she came to work in Patan, she had already become a well-known doctor by joining the medical movement,’ he said. ‘Even though she did not work in the same department, she was known as an energetic, agile and hardworking doctor. Later, she joined politics.’ If she sincerely joined political service, her potential in that field was high, Dr. Sharma pointed out.
