In Koshi Province, the upgrading of government health institutions and the opening of private hospitals have been increasing recently. The provincial government and local levels are operating hospitals at the district and municipal levels with their own investments. But government hospitals lack physical infrastructure, skilled manpower and equipment management. Despite continuous state investment, locals are not able to get quality services from problematic hospitals.

Despite the government-funded hospital being operated nearby, patients have complained of having to go elsewhere for treatment.

Along with the pressure of private hospitals in Biratnagar metropolis, government hospitals are becoming 'referral centers' due to disorganized management and lack of manpower. Koshi Provincial Hospital is suffering from overcrowding, cramped infrastructure, and cumbersome health insurance procedures.

More than 90 percent of patients at the metropolitan-level Rani and Baijnathpur hospitals come only to collect referral papers, making those hospitals look deserted. At the BP Foundation in Dharan, the online system is not effective, so the public is forced to wait in line for hours for everything from tickets to tests. There is an extraordinary crowd from city scans, labs to OPDs due to lack of equipment and manpower.

As the 'one municipality-one hospital' concept has not been effectively implemented in Sankhuwasabha and Khotang, citizens in remote areas are unable to receive even basic treatment. Rescue by helicopter is required. Delays in the construction of the hospital building, lack of specialists, and lack of equipment have made district-level hospitals weak.

Despite the capacity of 50 beds in Panchthar, service is limited to 15 beds due to lack of manpower and equipment. Even though magnificent buildings have been built in many municipalities, services have not started, due to which locals have had to travel to Dharan, Biratnagar, and even Siliguri in India.
