The fourth commission was formed 13 and a half months after the death of UML's then general secretary Madan Bhandari and head of the organization department in a jeep accident in Dasdhunga on 3rd June 2050. On the recommendation of the Cabinet, King Birendra announced the fourth inquiry commission on June 15, 2051 under the chairmanship of Supreme Court Justice Trilok Pratap Rana.

The fourth commission was formed by the government side, putting forward the idea that there is a legal obstacle to conduct an investigation under the chairmanship of the incumbent judge, as Amar Lama, the driver of the leader's jeep, was remanded in custody by the Chitwan District Court. 

How did the driver Lama cause the accident? Whether he was guilty or not, that matter was pending in the court . Therefore, it was the government's insistence that the committee chaired by the incumbent judge had formed a fourth commission to investigate Madan-dependent deaths on the condition that the committee chaired by the incumbent judge could not conduct further investigation.

Advocate Vipulendra Chakraborty and Harshanarayan Dhaubhadel were among the members of the three-member commission formed by the king. As Lama was imprisoned by the court, the king also consulted the judicial council about whether it would be possible to form an inquiry committee again. Regarding the commission's jurisdiction, it was said, "Except for the question of whether or not Netadwaya died due to motor driver Amar Lama, it will investigate other matters related to the Dasdhunga accident." The commission's working period was fixed for 45 days.

When the Supreme Court decided the Tanakpur case in 2050, Rana, who dissented, was made the chairman of the commission. After the

accident, the government formed an inquiry commission under the chairmanship of Prachandaraj Anil. UML accused the 'Government Commission' and rejected the findings and suggestions mentioned in the commission's report and formed an inquiry committee under the chairmanship of KP Sharma Oli, the then standing committee member of UML. The report released by that committee called the accident "premeditated and a conspiracy".

Madhav Kumar Nepal, the then general secretary of UML, released a brief statement through a press conference referring to the report of the Oli Commission. But what was written in the report? What is the conclusion of the report? Or the detailed description of what has been suggested has not been made public yet. 

Similarly, the commission of inquiry formed under the chairmanship of the left-wing leader Padmaratna Tuladhar as an independent commission on behalf of the citizens also rejected the judgment of the commission made by the government and gave its own conclusion. But even the commission could not give the reason for the accident. Rejecting the issues raised by the Anil Commission's report, UML was demanding an inquiry commission that would be accepted by everyone and trusted by its own party. He had been demanding that an inquiry committee be formed under the chairmanship of a sitting judge of the Supreme Court.

The UML was heating up the streets alleging that the death of its leaders was unnatural and that the Congress-led government had neglected to investigate it deeply. Due to the same demand, during the movement of UML, there was a huge loss of money all over the country . UML was very angry with the then Prime Minister Girijaprad Koirala . That is why unnatural slogans were being raised on the streets against Koirala in connection with the Dasdhunga accident. 

Kantipur Dainik published a news on 16th June 2051 under the title 'Formation of a commission to inquire about the Dasdhunga accident from Mr.

UML still hasn't resolved the matter of the leader's death in a jeep accident. The leaders have raised the question of investigation from time to time. Driver Lama, the only surviving witness of the jeep accident, was shot dead in Kirtipur on July 11, 2060.
