When a CRJ ship 'takes off' a 'pitch rate' of 3 degrees per second is required. But the investigation showed that the 'pitch rate' of the crashed plane reached 5.5 degrees per second that day. Strangely, the commission has found that in other flights of Solar Airlines, the aircraft is flying at a 'pitch rate' higher than the prescribed standard. It tells about the regular violation of standards and misconduct on the part of the pilots, it also shows the misconduct of Sourya Airlines itself.
Because, it appears that the company has not developed or is not serious about developing a system of internal monitoring and remediation of weaknesses. It can be suspected that there is a weakness or a lack of adequate orientation for the pilots themselves in terms of providing instructions or assistance for safe flight.
Likewise, there was great carelessness in the planning, preparation and operation of the 'Return Flight' to Pokhara, it was found that when loading the goods, the employees of Sourya put weight marks on the load sheet in Hachuwa without measuring the weight, the goods were not securely tied with ropes, the standards were violated, preparations were made in haste, highly flammable and dangerous chemical substances were kept in the ship's gallery.
