39-year-old Dr. Kamlesh Yadav from Birgunj has been the head of the district hospital for six months. In the 50-year-old dilapidated building, the district hospital, which used to perform only obstetric surgeries before, is now conducting major surgeries such as appendicitis, hernia, and hydrocele surgeries.

Dr. Yadav, a MDGP specialist, became the head of the district hospital and started performing complex surgeries.

Previously only obstetric surgery was done, but now the district hospital is a referral center for complex surgeries. Because the operation was done in another Bayalpata hospital in the district, now this hospital is going through a financial crisis.

Because of this, the number of patients in the district hospital has increased. Dr. Yadav said that the patients who used to get treatment from there are now coming to the district hospital. According to him, patients are coming here from Bajura and Dailekh.

'When I first came to the district hospital, it was very chaotic . Even if it is a complete building, we have managed it. Space has also been created for surgery," he said, "The biggest problem was in the store. It was not even possible to store the medicine because it leaked when it rained. We have been able to run the job by managing the small things . 90 percent of the physical management within the hospital has been done.

Dr. Yadav, who entered the eighth level through public service in July 2080, started his first field of work in the Far West. Yadav, who worked for 14 months after entering the Dadeldhura Hospital after passing the public service, then came to the District Hospital Achham.

10 years ago, he quit his job within 15 days after choosing to be a doctor in the Nepali Army. He has been working in a private health institution for a long time and has been serving in remote areas through government jobs for one and a half years.

"After joining the government service, I wanted to come to a remote area for self-satisfaction," he said, "It wasn't difficult to return after just working for a while." No one wanted a major surgery. I have insisted that what I have studied should be applied and patients from remote areas also get services.

He said that he has been conducting complex surgeries under his leadership from District Hospital Achham for the past 6 months. According to him, during this period, 50 cesarean sections, 3 vasectomy, 16 appendectomy, 6 hernioplasty, 6 hemorrhoidectomy (piles), 2 hydrocele,  87 major surgeries including circumcision 2 and laparotomy 2 have been completed. He said that a fee of 10,000 has been set for major surgery at the hospital.

The social security unit within the hospital will also give exemptions to those with weak economic conditions, the poor, helpless, and disabled based on the documents.  "Earlier, it was necessary to reach Nepalganj, Kathmandu to perform all these surgeries. Some of them were unable to afford treatment due to their poor financial condition. For such patients, it is a relief to get great treatment at a normal fee,' he said, 'It is satisfying to see happiness on the faces of patients in such a remote area using what they know and understand.'

He said that other services that can be provided are overshadowed because the hospital does not have a well-equipped building. According to him, there is also an orthopedic surgeon in the district hospital. But due to the lack of materials and buildings, bone surgery and treatment have not been able to expand . We are also managing for bone treatment and surgery. We will resume as soon as possible,' he said.
